2. Form 137-A (Kindergarten permanent Record)
Grade School Transferees (Grades 2 to 5 only)
1. Original Form 138 or Report Card
2. PSA Birth certificate (Photocopy)
3. Certificate of Good Moral Character
1. Grade 6 Report Card (Original and 2 Photocopies)
2. PSA / NSO-authenticated Birth Certificate (Photocopy only)
3. Certificate of Good Moral Character
4. Four (4) pieces of 2x2 picture in white background
5. Brgy. Clearance of Parents
Requirements for Incoming Grade 7
Junior High School Transferees (Grades 8 and 9 only)
